The University of Ibadan has announced the commencement of the University Health Service (UHS) Requisite Form for returning students seeking accommodation for the 2025/2026 academic session.
According to a notice released by the Student Affairs Division, all returning students who intend to apply for hall accommodation are required to complete the UHS Requisite Form within the stipulated time frame. The form serves as one of the mandatory requirements for consideration during the accommodation process.
The university stated that the UHS Requisite Form will close by 11:59 p.m. on Wednesday, 11 February 2026, and no extension will be granted. Students are expected to upload a scanned copy of their Jaja Card while completing the form.
It was further emphasized that final year students and those in concessional categories, including MBBS students and physically challenged students, must complete the UHS Requisite Form before they can be considered for accommodation.
The management also warned that any falsification of information submitted during the process will attract severe penalties in line with university regulations.
Meanwhile, the UI Portal for accommodation application will be open from Friday, 13 February to Thursday, 19 February 2026. Only students who have successfully completed the UHS Requisite Form will be eligible to proceed with the accommodation application on the portal.
Students are advised to complete the UHS Requisite Form promptly using the official link provided by the university to avoid disqualification.
